Health plans can't require prior approval for hepatitis C antiviral drugs

Health plans and insurers may no longer require prior authorization for medically necessary direct-acting antiviral drugs that treat hepatitis C. Where equivalent versions exist, a plan only has to cover one version that treats all genotypes without prior approval. Medi-Cal managed care contracts are excluded.
